Whether you're seeking support for yourself or for your child, your willingness to share vital information about the current situation is both appreciated and crucial. It helps us understand how we can tailor our expertise to meet the unique needs you or your child may have.

Step 1: Let's Get Started

Ready to begin your journey with us? Kick things off by filling out our Appointment Request Form. It's your opportunity to share your needs, whether they're for you or your child. Rest assured, your information is handled with the utmost care and confidentiality.

Step 2: We’ll Be In Touch

After you've submitted your form, our dedicated team will review it within 3 business days. If we believe we're a good match, expect a friendly call or text from us. We'll guide you through the next steps, including how to complete our Intake forms. which includes additional forms, consents and agreements for your review.

Step 3: Complete Your New Registration Form

Once you're ready, please schedule your appointment via the email we have sent you and fill the forms out on your portal including intake forms and consents. This is required prior to your appointment so our providers can plan for your visit.

Step 4: Insurance Verification

We'll collaborate with our 3rd-party billing partner or your insurance company to verify your insurance plan. This ensures a smooth process and helps us make the most of your benefits.

Step 5: Schedule Your Evaluation

We'll reach out to you prior to your appointment to ensure there are no pending items. Please be sure you are also checking with your insurance company as Three Rivers Mental Health cannot be responsible for your coverage.
